handling, the option to request we
delete their data file, and the choice
to opt out from utilization of that
information. To submit an opt-out
request, please fill in the form

Opt-Out Form
If you submit an opt-out request via email, you must provide the following information:
- Full name
- Full address (street address, city, state, zip)
- Accurate email
- 10-digit phone number
Please note: Your request will be processed within two weeks as long as we’ve been provided a valid email address for confirmation. It could take additional time before the full removal of your data is complete. The opt-out won’t apply to data that may have been provided to marketers prior to your request
